The Athletic Department will be hosting free sports physicals good for the 2019-20 school year on May 16 to any interested current 6th-11th grade student at AMS and AHS. In order to get your physical on this date all paperwork must be submitted electronically by going to alliance-oh.finalforms.com by April 30. No paper forms will be accepted. If you have questions or need a copy of the sign-up instructions please call the Athletic Office at 330-821-3972. Success Academy and Digital and Altitude Academy parents will need to contact the Athletic Office once their account has been created to be added to a team.
